LIVE NOW! Your Feedback Is Needed to Improve Patient Safety
Sent on behalf of Quality and Patient Safety
The 2019 Patient Safety Culture Survey is LIVE and ready for your input! Click here to access the survey online. Click here for a flyer.
As a reminder, this survey provides us with actionable information to support our mission of providing high quality, safe care for our patients and their families. Your insights and guidance are the keys to our success. In recent years, your feedback helped us implement patient safety rounds with support and engagement from executive leadership, increased Joint Commission Survey preparedness, “High Five” for hand hygiene program, patient experience of care training and nurse-driven protocols.
We are using an outside vendor, The Patient Safety Group, to administer the survey to ensure the results are completely anonymous. We want all of our employees to feel comfortable sharing their honest opinions about our patient safety culture and how we can improve.
While the survey should only take a short time to complete, the results can help create long-term improvements for our patients, their families, and our staff. Thank you for your cooperation and participation.

Crozer-Keystone Clinical Connections Program
Sent on behalf of Joseph Maloney, Assistant Vice President, Business Development
Crozer-Keystone Clinical Connections helps medical professionals quickly and efficiently transfer patients to a Crozer-Keystone hospital or Emergency Department (ED). This service can connect you with a hospital attending provider to help expedite your patient’s transfer, bypassing unnecessary wait times. Clinical Connections is available 24 hours a day, seven days a week. Medical transport is available for all transfers when needed. Clinical Connections can support the following:
Emergency Department Transport: Professionals from ambulatory practices and urgent care centers can call Clinical Connections to speak directly to a Crozer-Keystone attending ED provider. Patient information will be exchanged to expedite appropriate care upon the patient’s arrival. Clinical Connections can be used for non-emergent transport to a Crozer-Keystone ED. Patients transported to the ED will be taken directly to a hospital bay on arrival, bypassing the waiting room.
Direct Admission: If the patient’s condition does not require an ED evaluation, a provider can directly admit them to a service at any Crozer-Keystone hospital – without requiring the patient to visit the ED first. This service is available daily from 7 a.m. to 7 p.m.
Inter-hospital Transfer: We will facilitate the transfer of patients from any local hospital to the most appropriate Crozer-Keystone inpatient facility.
To access Clinical Connections, please call 877-243-8233. Click here for a print-ready PDF. Please share this information and help spread the word about this new service that we are now offering. Thank you.
