This commitment to quality is ongoing and multidimensional, encompassing the areas of health promotion, disease prevention, treatment of disease and illness, and restoration of function.

Patient Safety First

Crozer-Keystone Health System has committed to a continuous improvement approach to ensuring safe, effective and compassionate care for every patient. We focus on ensuring the best, most effective treatment, coupled with constant attention to identifying and preventing potential safety problems.

Patient-Centered Medical Homes

The National Committee for Quality Assurance (NCQA) recognizes Crozer-Keystone Health Network’s primary care practices as Level III Patient-Centered Medical Homes (PCMH). This is the NCQA’s highest designation for Medical Home Practices. To earn this certification, practices must undergo a rigorous set of standards set forth by the National Committee for Quality Assurance.

Comprehensive Primary Care Plus (CPC+)

Several Crozer-Keystone primary care practices have been selected to participate in Comprehensive Primary Care Plus (CPC+); a five-year program sponsored by the Centers for Medicare and Medicaid Services (CMS). CPC+ will help us to further transform our practices to meet the changing needs of the patients we serve.
